evaluation of documentation of multidisciplinary team (RN, PT, ST, SW, aide, etc) in a home health agency in los angeles.

Backgroung:poor clinical documentation compliance(78% out of 90%TJC ) by clinicians or MDT in a home health agency, using the agency audit tool, I want to evaluate global documentation compliance and per group of clinicans (nurses, PTs, STs, etc). the criteria to evaluate are: 

1- start of care visit within 48 hrs

2-evaluation from secondary disciplines within 5 days 

3-visit order frequency

4-nurse aide complinace in assignment

5-patient needs met

6-medication management

7-02 safety

8-plan of care

9-hospice care

10-discharge process.

P=multidisicplinary team(MDT) (composed of nurses, speech therapists, physical therapists, social worker, nurse aide, occupational therapist)

I= retrospective chart review to evaluate documentation compliance in the multidisciplinary team

C= compared to the 90% compliance established by the TJC and agency practice standards.

O=to identify gaps in the team documentation practice to provide recommendations.

* Sources need to be 5 years old 2016-2021, primary sources, and all need to answer each part of the PICO question. eliminate the ones that do not answer the PICO.

*Include the framework that answers the PICO the clinical performance feedback intervention theory (CP-FIT).

* i will attach a sample of the lit review.

*integrate articles, identify themes that emerge from the studies and gorup them in topics.

*Look at the best evidence: I need to convince the reader that doing cjart audits is very important for this A), B) and C). and it can help identify weakness in hte system and improve practice and patient care.

*include PRISMA methodoly of finding articles
