This assignment will assist in the development of competency in using a standard mental status report as part of the assessment of a clinical client. Additionally, it provides the opportunity for students to observe and analyze their process in assessment, paying careful attention to the potential for bias.

You will watch a brief video (see below) and then follow the instructions/outline to create a mental status report.

The assignment must be:

no longer than 3 pages.
Part 1 written in the past tense, Part 2 written in the present tense

The assignment can be:

single space within sections.
short phrases rather than complete sentences.

Instructions:

Write a mental status report based on the clip of the client in the video, using the report format below, and including each heading (A-K). Be sure to provide details to justify your conclusions by noting the specifics behaviors of what you saw, heard, and/or perceived. This will result in a more detailed mental status report than those typically written in clinic settings. No citations required.

Example: Maxmen and Ward note, the clinician “should present concrete illustrations to justify every conclusion. It is not sufficient to state that a client is delusional: the clinician should also report the content of the client’s delusions: “The client is convinced that penguins read his mind.” Saying the client has a depressed mood is a conclusion and as such requires substantiation: “The client moved slowly, cried throughout the interview, and never laughed. The client also said he felt sad most of the time.”

Part 2
Conclude your paper with a paragraph of reflection on your own process of creating a mental status exam. These reflections might address some of the following issues:

How did you respond subjectively (based on or influenced by personal feelings, tastes, or opinions) to this client?
Did you become aware of any particular assumptions or biases about the client in the process of writing this report? Where might these assumptions/biases have come from?
Were some sections of the report more challenging to address than others? If so, what made those sections more challenging?
