I have a lit review, this is analysing 5 research papers. The question I have chosen is;

AFFECTS OF ACUPUNCTURE VS SHAM ACUPUNCTURE ON PREGANANCY RATES IN WOMEN UNDER GOING IVF.

The Inclusion and exclusion must be the same in all the papers.

In preparation for the lit review, I am needing to make a presentation to my class, this consist of about 10slides;

AFFECTS OF ACUPUNCTURE VS SHAM ACUPUNCTURE ON PREGANANCY RATES IN WOMEN UNDER GOING IVF.

There are two types of infertility in a western medical perspective primary and secondary; the former is when someone who’s never conceived a child is having difficulty and the latter is when someone has previously had one or more children but is having difficulties conceiving again. There may be many reasons for infertility in women such as; Problems with ovulation, scarring from a surgery, Cervical mucus problems, fibroids, endometriosis, Pelvic Inflammatory Disease, sterilization and medicine and drugs. IVF is the means of fertilizing a women’s egg outside the womb and then replacing the fertilized egg (embryo) inside the women’s womb, this can be done with ones owns eggs and partners sperm or from donors. (IVF, 2021).
Obviously TCM doesn’t demonstrate the removal and replacement of eggs in its treatments of infertility but does have in own take on causes and treatment principal. The Pathology’s of infertility in TCM are as follows; Kidney Deficiency, Blood Deficiency, Cold in the Uterus, Dampness in the Lower Burner, Dampness in the Lower Burner, Blood heat, Qi stagnation, Blood stasis and Kindey Yin deficiency with Blood Empty-Heat. (Maciocia, 2019) . There are six stages in using TCM as an adjunctive therapy to IVF these are as follows; preparing the body to make sure the body is in the optimum state of health; Aiding the ovaries responsiveness to stimulation drugs; Care for the uterus after egg removal to ensure a healthy environment for the replacement; Assistant care for uterus on the implantation day ; After implantation using TCM to encourage successful implantation ; If fallen Pregeant continuing use of TCM to promote heathy conditions for the pregnancy. My original question I proposed was ‘The benefits of Acupuncture vs the Oral pill for the treatment of PMS symptoms’ however I found after searching data bases I could not find enough material that matched my search criteria to give me confidence I could complete my comparison efficiently. This search did help me arrive at my current questioning as I had an insight to the papers available for gynecological conditions. My final question key words were Acupuncture / Sham acupuncture/Pregnancy rates/IVF. The data bases I was using were ScienceDirect to which I received 93 results, one was suitable; ‘Randomized controlled trial; effects of acupuncture on pregnancy undergoing Vitro Fertilization’ found through ‘Science Direct’ on 18/11/2021. Participants had to be between 18-38 years old and on a fresh cycle of IVF. Women with a day 3 of Embryo transfer were Included, exclusion was women undergoing day 5 of embryo transfer, had prior experience with acupuncture or have previously participated in a trial. At this point I was looking or open access, I narrowed down the papers to exclude papers that included factors that were not in my criteria and were not open access.
From PubMed I received 10 results, 5 of which were systematic reviews which are unsuitable, one was measured on Live Birth instead of PR, two were outside the criteria and one was suitable to include if permission was granted by Elsevier publishing; ‘Influence of Acupuncture stimulation on pregnancy rates for women undergoing embryo transfer’ (Smith, Coyle and Norman, 2006), accessed through PubMed 24/11/2021, patients included those undergoing IVF. From Oxford Academic I found 20 results, with one suitable trial ‘ A Randomized double-blind comparison of real and placebo acupuncture in IVF treatment’ (Wing Sze So et al., 2008), published by Oxford University Press on behalf of the European Society of Human Reproduction and Embryology, assessed on 18/11/2021. The inclusion for this paper was that of patients that have a normal uterine cavity, exclusions were that of who didn’t and cancellation of ET due to failed fertilization or risk of hyperstimulation syndrome (OHSS). The other 19 were unsuitable, as they deterred off my chosen subject material or controlled outcomes. From the ASRM, Fertility and Sterility database I found 22 results from my key words, three of which were duplicate trials that I had already come across and included, there were several systematic reviews and meta-analysis which were automatically excluded and most that strayed from my criteria, the one I am using is . ‘P-45 The effect of Acupuncture on outcomes in Vitro Fertilization (IVF) (Udoff G, S, Z and McClamrock, 2006), found at ASRM, Fertility and Sterility. Inclusion/exclusion criteria; less than 40years old at start of stimulation, highest FSH less than 10mlIU/ml, less than 3 failed IVF attempts.
