Case Vignette 3: Darlene

You are a social worker at the Women’s Shelter, a non-profit agency serving women who are in need of protection from violent partners, and the children of these women. The agency is composed of a shelter for women without children and a family shelter with a wide array of counseling and support services (both individual and group) for women in the community who are not residing in the shelters. The agency prides itself on its outreach to ethnic minority communities and to young people through the teen dating violence program. You are new to the shelter and Darlene is your first client.

Darlene (age 29), a white female, first came to the Women’s Shelter some years ago after having run away from a violent relationship in another state. She met and married her present husband a year later and almost immediately had two sons. Within five years, the domestic violence in that relationship forced her to again seek refuge in the shelter, at which time Darlene filed for divorce. Her husband convinced her he would change and wooed her back with gifts and promises to seek help. However, he subsequently intensified his harassment and eventually attempted to kill her. Narrowly escaping with her life, Darlene filed charges against him for attempted murder, and returned to the shelter with her two young sons. Darlene discloses to you that she was emancipated at the age of 16 to escape a violent family of origin where she and her mother were beaten regularly by an alcoholic father. Darlene has a history of anorexia as well as alcohol and drug abuse. She is currently attending Alcoholics Anonymous meetings and has been in recovery for two years.

Darlene declares that she is now determined to leaver husband permanently and break the cycle of violence for the sake of her children. She still does not know whether he will be sent to prison. She appears mentally, emotionally, and physically exhausted and has confided that she is tired of living in fear, but “fear is all I know.”

Using the text to answer the following questions*: 

  1. As we’ve discussed in class, leaving abusive partners is difficult at best, and often it takes several attempts before success. Keeping that in mind, discuss the following:
    1. Societal perceptions of violence against women (keep a rural perspective), primarily women with substance abuse issues. Also include a discussion of your own biases related to violence against women and substance abuse.
    2. The challenge of realistic goal formation with clients and identify 3 or more strategies to form goals and objectives for balancing vision with reality.

  2. Develop a list of the most essential areas to explore at intake with Darlene:
    1. Include questions and mock dialogue that shows how you can approach building trust and rapport.
    2. Discuss 3 therapeutic (e.g., CBT, ACT, Solution Focused) and/or mindfulness techniques that you can employ with Darlene to help decrease and manage her anxiety. Explain your reasons for choosing these methods.
