Presenting Complaints: (10 points)

Symptoms are described with an adequate amount of detail to inform diagnostic decision making (i.e. duration of symptoms, description of cognitive, emotional and behavioral symptoms).

Current symptoms are described in a logical pattern (i.e. depressive symptoms then anxiety, then psychotic, etc.)

Appropriate domains are covered or at least screened for (i.e. activities of daily living, suicidality, substance, psychotic symptoms, trauma, sleep, etc).

Medical history; Family History; Developmental/Social History; Educational/Occupational history; Behavioral Observations: (Discuss any of these sections as relevant: 5 points)

            Possible factors/stressors are described or ruled out in an appropriate amount of detail to inform clinical decision making.

A brief example, “Johnny denied a significant family history of psychiatric, neurologic, or other serious medical conditions.”

An example requiring more details, “Johnny reported recently starting a new job (continue to describe possible stressors, timelines and impact on everyday life).”

 

Diagnostic impression: (10 points)

            Symptoms described in Presenting Complaints lead to DSM-5 diagnoses. For example: “Johnny reported symptoms of depressed mood over the last two months. He additionally, reported symptoms of XX, XX, … and XX. Symptoms impair his daily life in terms of… Therefore, he meets criteria for Major Depressive Disorder (MDD), mild, single episode.”

            Additionally, possible diagnoses are ruled out as appropriate. For example “Johnny reported mild difficulties with concentration and inattention, which may raise the concern of a possible ADHD diagnosis. However, symptoms were primarily attributed to MDD, therefore he does not current meet criteria for ADHD.”
