The major assignment for the course is a group assignment where the groups that worked on the resource guide will work collaboratively on the development of a complete grant proposal based on a public/community health need. Students will have two options for which they may write a proposal. Those options are available separately in the “Request for Proposals” document. The full proposal must include the following elements:
1. An APA formatted cover page (no cover letter)
2. A 250–300-word abstract of the proposal (following provided guidelines)
3. A table of contents showing major parts of the proposal and the corresponding page numbers
4. An introduction that includes background, organizational overview, and experience
5. A well-developed and compelling problem statement that draws on empirical research
6. Proposed strategies/methodology for addressing the problem, with due consideration of alternatives
7. Clearly articulated (SMART) goals, objectives, outcomes, and impacts
8. An evaluation protocol that includes formative and summative approaches
9. A fully developed reference list for all sources utilized (minimum of 10 academic, trade and other sources)
10. Appendices that include a “sources and uses” budget (required), a logic model (required), tables (where necessary), and other documents relevant to the proposal (optional)

- Part 3: Evaluation Protocol: Formative Evaluation Plan; Summative Evaluation Plan; Relationship to Goals/Outcomes
- Part 4: Budget; Budget Narrative/Justification; Logic Model, Tables
The assignment must have an APA formatted cover page, an abstract, and a table of contents, and the required appendices. Formatting must be consistent with the requirements of the Publication Manual of the American Psychological Association, 7th edition. Pay particular attention to proper citation and reference format, subheadings, spacing, etc.
