Apply a knowledge base of community health nursing and health teaching skills to the development an educational project with a focus on illness prevention, health promotion and/or health maintenance of individuals, families and groups.

Throughout this course, each module assisted with the creation, presentation, and reflection of a community health project for community health nursing. In my project on diabetes management, I was able to utilize my teaching skills, focusing on prevention, promotion, and maintenance of diabetes for those who have already been diagnosed. However, family and caregivers were also invited which has the potential to impact individuals, families, groups and the community as a whole.

Demonstrate use of information technology in data gathering and analysis of a community or a specific population group.

Information technology was an integral part in my data gathering for my project. The majority of my information required the gathering of local, state and national statistics to determine the prevalence of diabetes and how it impacts the community. Due to the current climate and some restrictions with my location of choice, the presentation was also conducted using information technology for communication and video conferencing.

Analyze a community assessment to plan a health teaching project that meets the needs of communities and populations

As per module two, I was able to utilize my community assessment from Community Health Nursing I to establish the foundation for my current project. This information allowed for me to determine the needs in an effort to meet them throughout the presentation.

Create an educational project that can be utilized by public and community health nurses to improve the health status and eliminate health disparities of individuals, families, communities and populations.

I was able to create and educational project that can be utilized by public and community health nurses, however, it will have to be tweaked a bit in order to fit the timeframe as well as offer additional modules and follow up with participants to ensure that the needs are effectively met.

Implement the planned teaching project to the selected target population.

I was able to implement the teaching project as expected. The participants were very receptive and provided good feedback. Many prefer to offer additional information as well as participate in subsequent education should it be provided.

Collaborate with community partners to provide education designed to improve population health.

I was able to effectively collaborate with the Hillsborough County Health Department despite the restrictions placed within the current climate and produce a product that met the objectives of the project. Although the Health Department was my primary collaborator, I was also able to collaborate with community churches and utilize social media to recruit participants.

Record and present evidence of the completion of each phase of the teaching project

I was able to record presentations for both module 7 and 8 that provided information regarding the stages of development and presentation of the project to the target audience. With the presentation of evidence, I was also able to incorporate evaluations from myself and the participants.

Syllabus:

N493 Community Health Nursing II builds upon the foundation provided in N492 Community Health Nursing I. From a theoretical and scientific framework, the concepts of epidemiology and disease transmission, comprehensive assessment of risk factors and health problems, program planning and intervention, environmental health, and collaboration with the interdisciplinary team are explored. The practice experience component focuses upon developing and evaluating health promotion programs, family assessment, community assessment, and community-based home care within the context of the community. The nursing process is applied with the goal of promoting and preserving the health of populations. It is anticipated that 70-100 practice experience hours will be required to complete this health promotion project. **Please note, students who hold an RN license in WA are required to complete a minimum of 80 practice experience hours in this course.
