1.(250 words) Review the topics for this course as outlined in the course syllabus and course modules. Provide one concrete example of how at least one topic relates directly to your current practice. Then, discuss at least three learning goals you have set for yourself for this course.

The content areas what would apply to me is special populations. In my current practice as a case manager, I plan discharge with patients who often require alternative plans. Most of the patients are elderly and have multiple medical problems. Also most have issues such as polypharmacy.
The 3 learning goals I set for myself during the course directly relate to this learning module:
Explain the steps in planning and evaluating health care programs
Identify determinants affecting the health of the elderly.
Analyze the role of population health nursing with respect to the elderly.
