The Community Health Improvement Partners (CHIP) is a voluntary collaboration of San Diego, California healthcare stakeholders including hospitals, community clinics, insurers, physicians, universities, community benefit organizations, and the County of San Diego. The members of CHIP are dedicated to the common vision of “achieving optimal health for San Diego Countyâ€s communities through collaboration and assessment.†Formed in 1995, the CHIP model is a community health improvement model to measure performance and drive policy development.
