Health History Assignment
Purpose: To gain experience in gathering a health, family, and genetic history to identify current and potential health risks.
Assignment:
With the help of a friend, classmate, or family member, complete a comprehensive health history interview using the Nursing Interview Guide for Chapter 2 (under skills mastery tab in the Weber & Kelley Course Point +online, also available in this document below). This interview should be conducted as if a new patient is presenting to your clinic for an entry physical. Utilize your cell phone, iPad, computer, etc. to record yourself conducting the patient interview.
Following completion of the interview watch the video recording and analyze your body language, tone, speech, and overall communication with your patient. Utilize the document provided in D2L labeled 10 traps of Interviewing (located just below these assignment instructions) as well as your textbook to guide your evaluation. When you have finished viewing the recording, complete the reflection questions below. Your video recording of the interview will NOT be turned in.
1. Your completed Health History Write-Up (Attached), which should be typed or neatly written and scanned. Please be sure to use appropriate medical terminology when documenting your patients information. Responses should be succinct but provide adequate detail to create a clear picture of your patients health status.
o TIPS:
Please utilize initials for your patients name and leave the address and birthdate blank.
With the exception of male vs female categories, all information should be completed and documented.
Normal is not an acceptable response.
Explore abnormal responses to create a clear picture of your patients health status.
Ask yourself, is NA or Not Applicable the appropriate documentation? (For example- if the patient has no past surgical history should that category be documented as NA or patient denies past surgeries)
2. Your typed or written responses to the reflection questions below.
o Responses should be in complete sentences, utilizing appropriate medical terminology with no spelling or grammatical errors. Failure to follow these instructions will result in a zero for the question. (5 points/1 point per question)
o Responses should address all components of the question in a thoughtful, thorough and professional manner utilizing concepts covered in the provided/suggested resources should be evident and included in your responses.
