1. Kara, a therapist on the county hospitals psychiatric unit, is a recent graduate from a masters degree program in mental health counseling. She completed her practicum while working in a treatment facility for troubled children and adolescents, and her internship was done at a privately owned psychiatric hospital for adults. She was recently offered a position as a dual-diagnosis treatment counselor at a local drug and alcohol rehabilitation facility. The position pays much more than shes making at the community agency, and is within ten minutes of her home. During the interview, she was told that she would need to work independently, with only limited supervision. Kara then contacted the facility and let the supervisor know that she is not able to accept the position at this time, but that she plans to seek out the necessary training and supervision to work with substance abuse clients, and that she hopes the facility will consider her for the position once she has completed the necessary training. 2. A therapist had an intake appointment with a new client. As soon as he looked at the intake form, he realized that she is the ex-wife of his former client. The therapy with the ex-husband was brief, and he focused on how he wanted to leave the marriage. The prospective client was using her maiden name and gave no indication during the session that she knew about her ex-husbands treatment with the therapist.
When asked about the reason for the referral, she said that her physician had referred her for anxiety. She mentions that she is still in communication with her ex-husband because they share three children. The therapist informed the new client that he would not be able to work with and offered her a referral to a colleague. (3pts)

V1: Sabine, age 14, is the oldest of three children. Sabine has missed school for 1 month and is refusing to go back. She has been referred to your counseling agency by her school Guidance Counselor because her mom is worried about some of the dark things Sabine has been saying. According to referral summary from the Guidance Counselor, Sabine sent her boyfriend some pics a few weeks ago. Sabine reports that she I thought he was my boyfriend, but he showed them to his friend and his friend sent them to everyone. The school found out and now the police have spoken to him and his friend.
Sabine has not returned to school since the incident occurred. Sabine reported to the GC that she is being called a slut on social media by classmates and their friends. Her mother, who has accompanied her to this initial counseling session, reported to the GC that Sabine keeps saying I cant deal with them looking at me and I know what theyll be thinking. Even the girls know all this detail about me.
The stupid thing is, everyone does it, everyone sends pics, but I was just unlucky to have a boyfriend who betrayed me. I will never trust anyone again. I feel like everything is over and theres no going back now.
V2. Michael is 22 and works with the big e-commerce distribution center in his town. He was referred to you by the Employee Assistance Program at his place of employment. Per the referral, He had been dating a coworker for a year, and thought things were going really well. He asked her to marry him, however, she said she wasn’t ready for a commitment and instead she went overseas – on her own. Since then, he can’t stop thinking about her and how hurt he feels. He hasn’t been able to sleep. He’s been out drinking every night with his friends. He’s been feeling like a ‘pent up volcano’. His coworkers have started to hassle him a bit about ‘not being on the job’ and last week, when the foreman yelled at him about something he started screaming back and almost threw a punch. Human Resources Department at his company has directed him to take some time off and provided him with an Employee Assistance Program pamphlet; which is how he found your agency.
 
 
V3. Liz is a 28-year-old financial analyst who has been in her job for several years. Previous to this current job, she held several part-time service industry jobs while going to school. Liz had an active social life and, while she partied less than she had in college, she would still occasionally come to work hung over and/or having had very little sleep. She would also regularly find herself recovering on Sunday in preparation for the work week ahead. For the most part, however, Liz had confined her partying to the weekends, drinking very little during the work week, until her mother was diagnosed with terminal cancer several months ago. Since that time, Liz has been having two to four drinks almost every evening: more when she is at a party or social gathering. Hangovers and sleep disturbances are more frequent, but Liz cant imagine coping without the alcohol to ease the pain
One day, her supervisor asked Liz how she was doing and she unexpectedly burst into tears. She ended up telling the supervisor about the situation with her mother and it turned out her supervisor had gone through a similar situation several years earlier. Lizs supervisor also suggested that she check out short-term counselling as a resource for further support. After a week of hesitation (and a few more hangovers), Liz called your agency.
