~Indicating if you have math anxiety (use test results) and include your score
~Outlining specific strategies for relieving math anxiety, and
~Identifying specific techniques for taking notes in a math class.
Note: If you are fortunate to NOT have math anxiety, please include methods you will use to be successful in this course. If you DO have math anxiety, explain some techniques you plan to use to reduce it.
