Noticing: One thing I noticed while giving medications was that my patient was prescribed two diuretics, which are both known to lower blood pressure. After reviewing the chart, I noticed the nurses’ were giving them at the same time days prior and felt they could have been more cautious given the patient was already on the low end of the acceptable minimum blood pressure range (90 over 60).
Response: Instead of giving them both at the same time, I waited one hour in between before giving the second diuretic. I checked the patient’s blood pressure to ensure it was within normal limits before giving the second medication.
